Evaluation Process
Evaluation
*Request that your school district fund an independent transition planning evaluation.
*Transition planning evaluations are best done collaboratively between parents/guardians, the student, and school staff.
*Sign consent for the evaluation at the time it is approved.
*Private pay available on a case by case basis.
* Parent/guardian interview.
*In person, onsite evaluation of student in their current school program.
*Interview with team members.
* Parents will be provided with a set of assessments to complete and return regarding their child’s current level of performance in the transition planning areas.
* School staff will be provided with a set of assessments to complete and return regarding the student’s current level of performance in the transition planning areas.
* FYI: failure of people to return assessments is the #1 cause for delayed completion of the evaluation. Please ask questions whenever needed.
* Completed once all information has been provided and assessments have been returned.
*Recommendations in each of the transition planning areas are made for team consideration.
*Parent conference and attendance at IEP meeting to present results if requested (virtual).
